What is the main goal of your services?

We provide services to Medicaid individuals with persistent mental illness. We establish prompts and routines to ensure they are successful in independent living.

Who is eligible for services?
  • Individuals with a mental health diagnosis
  • Over the age of 21
  • Medicaid Insured
  • Compliant with mental health clinician
  • Seeking assistance with a minimum of 2 IADLS
How long does it take for someone to receive services?

Our first initial outreach for the intake will typically be a month out from when the initial referral was submitted. DSS staff pairing is dependent on staff availability and client location.

Are you able to provide personal care?

Often times Northwest Habilitation receives requests for caregiving services. Although there may be overlap for some of our clients, we are not able to perform personal care tasks. It is the expectation that a client is able to navigate medical and hygiene concerns either independently or with other members of their care team. Our role encourages prompts and reminders for individuals that may be seeking organization, routines, or motivation to complete daily living tasks.

Do you provide housekeeping services?

We work with clients to create routines and systems around housekeeping. Clients are expected to participate in the services as well. We do not perform housekeeping tasks on behalf of the client.

How many hours are available to clients and what is the typical length of a shift?

Clients typically have up to 20 hours to use weekly and the average hours for a shift will be around 4 hours per day.

Are you able to drive clients using your own vehicle?

No, DSS vehicles are not available for use. While on shift we will help clients use their natural resources available for transportation, including TriMet and ride-to-care.
