What documents are needed?
Who should fill out the packet?

Someone on the referred clients care team that is a QMHA or higher. This can be filled out with the referred client.

How should I send the documents?

You can send documents to pdx@nw-hab.com. 

An ROI is included in the referral packet so that MHA and treatment plan can be shared.

What are the next steps after the referral is sent in?

Our onboarding specialist will submit documentation to Comagine directly. Once approved, we will reach out to client to schedule intake.
